چکیده

Midgut volvulus as a result of malrotation classically presents in early life and is associated with intestinal obstruction and compromised vascular supply of the gut. On rare occasions this condition may manifest itself in teenagers and adults, often with disastrous outcome. We report a 28-year-old patient who presented with maelena and endoscopy showing varices in the duodenum and large bowel. During the neonatal period of his life he had been operated on for intestinal obstruction, but the details of the operation were unknown to us. Angiography demonstrated superior mesenteric vein thrombosis and ectopic varices. On laparotomy he was found to have midgut volvulus. Recommendations for diagnosis and treatment are discussed.

Small bowel varices secondary to chronic superior mesenteric vein thrombosis in a patient with heterozygous Factor V Leiden mutation: a case report

INTRODUCTION Bleeding ectopic small bowel varices pose a clinical dilemma for the physician, given their diagnostic obscurity and the lack of evidence-based medicine to guide therapy. They often occur in the context of portal hypertension, secondary to either liver disease or extrahepatic causes. Rarely is their presence associated with chronic superior mesenteric vein thrombosis and hereditary...

Clot dissolution: a novel treatment of midgut volvulus.

Midgut volvulus due to malrotation may result in loss of the small bowel. Until now, after derotation of the volvulus, pediatric surgeons do not deal with the mesenteric thrombosis, which causes continuing ischemia of the intestine. On occasion, a "second look" laparotomy is performed in the hope that some improvement in blood supply to the intestine has occurred. We describe a new combined tre...

Ectopic varices: Hidden challenges in the vascular anatomy.

A 50 yo female with PSC and a bleeding duodenal varix (DV) was evaluated for possible enbucrilate therapy. Endoscopically (Figure 1), the DV was high risk for rebleeding but the relative in-flow and out-flow were uncertain. CT (Figure 2) showed portal vein thrombosis (PVT) and the vascular anatomy of the DV to consist of inflow from the superior mesenteric vein (SMV) with outflow through the DV...
